Passed away peacefully at his home 8/3/04
Aged 66 Years

Dearly loved husband of WANDA, much loved father and father-in-law of CHRISTINE, MARTIN and CATHERINE and loving step-pop of JARRAD, MATTHEW, SHANNAN and KRISTY and beloved brother and brother-in-law of PHYLLIS and the late ROBERT WATT, MARIA and the late PHILLIP DUCAT, BRIAN and JANET HITCHCOCK, DON GOLDRICK and fond uncle of their FAMILIES.

The Relatives and Friends of JACK are warmly invited to attend his Funeral to be held in the Chapel of Lake Macquarie Memorial Park, Cessnock Road, Ryhope (Toronto exit off freeway) FRIDAY (12/03/2004) Service commencing at 2 p.m.
