Passed away Wednesday 31st March, 2004

Relatives and Friends of NEIL and LIZ BOLLEN, LOUISE and SCOTT KOZARY, SIENNA and REMY, and KRISTEN BOLLEN, are invited to attend the Funeral Service for their dearly beloved mother, mother-in-law, Nan, Great-Nan and aunt ENID to be held in the David Lloyd Funeral Home, 200 Brunker Road, Adamstown MONDAY afternoon 05/04/2004 commencing at 2 o`clock.
A private cremation will follow this Service.

The Family wish to thank the Palliative Care at the Mater Hospital, Waratah and Community Nurses from Toronto for their wonderful care and support.
